Tahoe starts to die?

Hi. I'm new here and I got a question. I have a 97 tahoe with a 350 vortec. The truck itself has 137k miles on it but I had the engine and transmission professionally rebuilt by GM at 132k. Sometimes when I start my truck, it idles fine, but when I shift into gear and start to move, the RPM drops down to like 400 unless I either let off the gas or floor it, and if I keep on the gas only enough for normal acceleration, it dies. It happens once or twice a week, whether I let it warm up or not. After it bogs down once, it doesn't do it again.

I have replaced the fuel pump, fuel filter, had the injectors cleaned, new air filter, bosch platinum spark plugs and run castrol gtx oil. I run mid-grade (89 octane) gasoline.

Is there another issue I should be looking for to solve this problem? It's not a huge issue but it makes me worry that it could get worse down the road. Any help would be appreciated.

I bought it with 127,200 and it wasn't properly maintained by the previous owners. It had shifting problems, burned oil, had a leaking head gasket, and lifters stuck badly, so I decided to just do the whole thing. They kept it clean and detailed but apparently didn't care about how it ran. I'm unsure about the codes. I normally have someone else do something like that for me. I do light and moderate work but never really came across that knowledge.

Nice..... kinda opposite my philosophy, keep it running top notch, not so worried about the body......
