2003 Tahoe Front end Vibration when braking

I recently replaced my brake pads and one rotor on my Tahoe(had the other rotor turned). Ever since then when I am going 55mph or faster and brake I get a violent vibration in the front end and pedal. Sometimes the ABS will kick on. The rotor that was replaced was so bad I had to cut it off and knock it off with a sledge hammer on the passenger side of the truck. I verified that none of my Brake cylinders were froze up and I made sure I had no playing in ball joints etc.

Either damaged the hub when you used a sledge hammer, machined rotor is too thin and warped already or the new rotor was warped right out of the box.

Thanks for the replay. I kind of wonder if the hub has surface rust on it from the old rotor that maybe i need to clean off?
